When requested by the County Social Service Agency, or the Department, any of the following information and verifications must also be provided:

Fire inspections by the state fire marshal or local fire inspector
Physicians examination
Psychological examinations
Proof of age and relationship of applicant and/or individuals residing in the home
Sanitation and safety inspection reports
Drug and alcohol evaluation report
